UPDATED FORM POLICY

FORM FEES:

The office charges a fee for all paperwork that requires completion by our providers. There will be a $5-10 fee for all standard forms (universal health record, sports physical form, asthma action plan, food allergy forms, etc) not completed at your child’s checkup.

 

School or work accommodation letters, FMLA forms, disability paperwork, and other miscellaneous letters are a standard fee of $25. The provider will take 5-10 business days to complete the letters. 

 

DMV forms such as handicap placards or license plate applications, social security letters, and primary diagnosis letters are $5.

 

All standard forms will take 2-5 business days to complete. Letters, including FMLA and disability paperwork, will require 5-10 business days to complete. An addtional fee of $ 15 will be charged if the form must be completed the same day. For convenience, forms can be requested and submitted via the patient portal. Once completed, forms will be uploaded to the patient portal.

 

Please write the patient’s name and date of birth on each form. Any form submitted without the child’s name will be discarded. We will NOT fill out any forms for medical clearance if the patient has not had a physical exam within the past year or if the child has never had a physical exam visit at our office.

 

Changes to any forms that have been completed and submitted are subject to an additional fee.

MESSAGES:

Dr. Bob requires all messages to be initiated via the patient portal. Urgent questions regarding acute illness (i.e., fever, vomiting, sore throat, etc.) should still be answered by a phone call to the office for proper triage by our staff. Non-urgent, chronic, behavioral, emotional, or general questions should always be initiated via the patient portal. Dr. Bob may still call you to discuss these matters. However, he prefers that all conversations be initiated via the portal. This allows for quicker responses and more consistent contact.

NEW PATIENT ANNOUNCEMENT

As of February 1, 2024 we will  be accepting new patients. The doctors current schedule is limited, first time visits must be seen with Dr. Christine Malaty to established your children with the office. We will continue to welcome expecting [new first time families] newborns.

FACE MASK

Face masks will be required for all sick visits, curbside included, for the foreseeable future as we continue to see covid-19, flu, and other highly transmissible illnesses.
